Business Card Request

All requests must be made with at least two weeks notice prior to the required arrival date, unless managers have provided prior permission to express mail.

Guidelines for Business Cards

• Nicknames are acceptable provided they match your information in ADP.
• The job title listed on your business card must match ADP exactly.
• To include education and/or credentials, proof of education and/or credentials must be sent to HR for your personnel file.
• You must use Empire Genomics or Biocare Medical provided email address and phone number. No exceptions.
